Safety Report
The survey will take approximately 8 minutes to complete.
This safety report will be treated confidentially, and will be brought in discussions with Indonesian aviation stakeholders to improve safety In Indonesia.
There are two ways of the report:
A. Hazard Report that may affect the safety of Flight Operations.
B. Mandatory report due to Incidents or Accidents. This type of report should be sent directly to the authority, however if the crew involved feels the need to do it confidentially, choose this.
